'see me' Action Planning Tool

Purpose

The ‘see me’ Action Planning Tool builds on the “getting started section”. Here you’re supported to develop your plans further. This tool provides prompts to support you to further clarify: your aims and objectives, your target group or groups and planned actions. You are supported to identify current local opportunities and develop strategies for overcoming challenges. A timetable for action helps you to be realistic but also increases the likelihood of you achieving your goals.

Content description:

  • 1. Aim of the action or actions (try to write a clear brief description of what will be delivered, why, by whom, where and by when).
  • 2. What and who is included (what’s included in the actions such as target group, geographical areas, places i.e. community, schools, workplaces).
  • 3. What and who is not included (it can be helpful to think about what is not included as a way to focus your actions).
  • 4. What will your actions achieve (what will your actions achieve, how will you know you have achieved them and how will you share your learning).
  • 5. What opportunities and challenges exist in relation to your planned actions (budget/time/resource/supports).
  • 6. What are your target dates for your actions (short, medium and long-term).
  • 7. How does your actions link with and support local and national aims, objectives, polices and priorities. (understanding this helps make the case for action).
  • 8. Who needs to be involvement (who needs to be involved, consulted and informed).
  • 9. What information sources already exist supporting your action (evidence base, information systems, published reports, experts by experience).
  • 10. Why have you chosen this action (this helps you think about other actions that may help you achieve your objectives).
